Any Shenandoah student must have all of the following before practices begin for the Fall Sports:

1. Physical on file in office
2. Student AND Parent drug consent form (Harmony registration)
3. Student AND Parent concussion and sudden cardiac arrest form (Harmony registration)
4. IMPACT concussion testing (freshmen and juniors)
